Logo for Professioonal Business cards and business card design

This is what CPRN,LLC, I am sending my ideas of wording, however, I need advice and recommendations on this. I need advice on setting this up to look professional, colors , will not have my address, but need my Name, credentials, phone num, email and whatever you advise for a professional business card and log that can then be sued for invoices, stationary and certification cards down the road. I am a Registered Nurse, A Certified Red Cross CPR instructor, and Case Manager.
