App Design Brief

This is a design of 5 icons and main interface theme for an iphone/android app that will be used to assist people with managing and sharing their medical information.

I am an independent software developer and this software has the potential to save lives so I'm personally passionate about it. The basic abilities of the program will be provided to everyone for FREE!

Icons must be developed in Adobe Illustrator as they will be used in printed materials.

1 icon represents scanning/entering/photographing prescription information into the app to build a list of medications
1 icon represents notifications both emergency and non-emergency via text, phone, email
1 icon represents the codes status (patients wish for resuscitation etc.
1 icon represents legal forms and documents
1 icon represents personal medical history

I'm including a figure that shows the interface as I have put it together so far but it is not the professional "themed look" that I want.

3d icons would be interesting... a background is ok, you can use words if you like
