Project: LTPAC HIT (Long-Term and Post Acute Care – Health Information Technology) Collaborative Logo/Identity
Project Summary:
LTPAC HIT needs a logo/identity designed. It presently uses a generic icon pulled from icons available on-line, and which is used by many other organizations.
Introduction:
Long Term Care (LTC) is a variety of services which help meet both the medical and non-medical needs of people with a chronic illness or disability who cannot care for themselves for long periods of time.
LTC providers focus on:
- coordination of supportive services and care
- restoring and maintaining health
- wellness and functional abilities
- meeting the needs and goals of each person receiving care and their families
This focus requires healthcare technology to facilitate:
- shared care and transitions of care and
- person-centered longitudinal health and wellness records,
in order to ensure a person receives affordable, quality and coordinated care.
Examples of LTC providers include:
- home and community-based care services
- nursing homes
- assisted living
- long-term acute care hospitals
- rehabilitation and post-acute care facilities
- hospice
- chronic disease and co-morbidity management
- wellness providers
Background:
Founded in 2005, LTPAC HIT is a collaborative committee with representation from several healthcare organizations.
LTPAC HIT Vision: Enhancing quality in long term/post acute care and services through the meaningful use of health information technology.
LTPAC Mission: working as an industry with government policymakers to ensure resources, technical standards, and provider knowledge are aligned with the nation’s growing need for long term/post acute care and services.
In summary, LTPAC HIT collaborative is a group of stakeholder associations who recognize their common interests and vision for health information technology (HIT). The collaborative was formed to advance HIT issues through coordinated efforts, hosting of an annual LTPAC HIT Summit and publishing of a document that provides guidance on HIT issues.
A logo/identity is needed, and must be flexible for use on website, and in electronic and print documents (including presentation materials).
