Logo Design Brief

I need a logo, letterhead, referral pads and business cards for my start up practice. I am a dental specialist- Periodontist. I deal with gum disease, gum surgery and surgery placing dental implants.
The name of my practice is "Northern Periodontics & Implants".
The logo should fit letterheads, referral pads and business cards (and even stationary slips. I like a pleasant blue and mainly whites. I like the logo on www.periogroup.com.au (i work there) and whitelight prosthodontics (in Melbourne Australia).
The business card should have logo one side, my name and qualifications on the other side and lines to fit appointments for patients.
Referral pad is A5 Letterhead is A4.
I like files of pdf,EPS,JPEG and bitmaps so I can use in signage, print medium and computer presentation.
I have attached some requirements for the A5 referral pad and also for A4 letterhead, rough layouts only
