Hi guys,

That sounds great. Raju, I am not sure what you mean by the list. I am looking for referral form design which is typically cut longwise across the 8.5x11  paper and can fit 2 per regular sheet. Rest needs to be an envelope design, stationary paper, paper pad, thank you letter, fax, etc. Hopefully this helps. As far as sizes just standard except for the referral form which is typically 4to5 x 8.5 inches. Referral forms are double sided with the information I provided. We can definitely adjust these as needed. Let me know if this clears anything up Raju.

I think the list is to help us understand how many type of collaterals need to be design with the limited budget. If 4 types for example, for 200, each design cost will be 50.

Sometime good design takes time and time is money, with very small budget, it might not interest many good designer. Perhaps increase the budget or less collaterals.

It just a suggestion, thank you.

The logo in the EPS file, and the one on the business card - are different in terms of colors used. Which is the finalized logo?

You can definitely play around with the different shades but I think the darker one on the business cards is the one I ultimately will be using just as it prints somewhat better on regular paper and in black / white.

I just thought of a change that we may be able to incorporate into the design. Instead of having the drawing of all adult teeth. Somekind of a check off system will work.

However, just like the soft tissue chart incorporating a picture of the upper and lower jaw without teeth that in case of a bony biopsy they may be able to draw on.. 
I will try to find a picture of what I am thinking of. 
Something like a translucent version at the very bottom of this page.
It would probably just have to be a drawn version so that it prints well.
