I need a simple app for Iphone so that my employees can send me the details of where they worked and how many hours they worked for. I have a logo that will need to be integrated into it.
We will need it to also email the information to a centralized address so that the invoicing and wages can be done.
It will need to have the

Mine Worked (This will have a drop down menu with 7 mines listed)

Vent Report Number
Emergency Response Report Number
Surname
First Name

Target Market(s)

This app if for employees of the company, all fields will need to be filled out, but they can have either a vent report number(V/R) or and Emergency Response Report number (ERC)
