A broken appointment is a loss for everyone. Please inform us 24 hours
in advance if you are unable to keep your appointment or you may be 
charged for a missed visit. 

This is all the information that needs to be on the card. Obviously the font sizes and 
such can be different to make everything flow well and be presentable.
