Better Health Plan for the West is a partnership of twenty different health organisations in the western suburbs of Melbourne, working together to improve the health of the local community. The western suburbs of Melbourne is one of the fastest growing and most culturally diverse populations in Australia and its communities have some of the poorest health outcomes in Australia.
