Letterhead Design Brief

My husband and I are starting our own practice named TK Ortho & Wellness Center. We would like to create a nice simple but high class letterhead that doesn't use too much solid color ink when printing. I like the colors dark blue, gold, yellow. Open to other suggestions. We will need to list him and I both on the top of the letterhead. Here is the important information:

The only thing is that the phone and fax numbers will change once we get into our space. I just want to get the design started. I am including several documents which are my previous practice letterhead, my current procedure room letterhead, and our new Logo. The other letterheads are just for examples but please think outside of the box! We would love to see some great designs.
