Magazine Design Brief

Hello, Please help me design a half page Ad for my medical practice here in South FL.
I run a private, Concierge Primary Care Practice in Coral Springs, FL - I'm attaching other designs I've had created from DesignCrowd & I'd like the aesthetic of this magazine Ad to match them.

I'm also attaching the specifications per the magazine publisher.

Things that I'd like you to definitely put into the Ad:
- My photo (attached)
- Our contact info (as on the business & rack cards)
- Title: Concierge Direct Primary Care
- Subtitle: Primary Care without Insurance Obstacles
- A "mini" version of the back of the rack card (attached) using the services that are listed but also include "On-Site Lab" + "Some Medications-Dispensed On-Site"
