Letterhead Design Brief

I receive a fantastic logo from you guys and I'd like to incorporate it into business cards and letterhead. I accidentally approved design No. 32361474 and contacted designer Manish, and Manish sent me the zip folder for Design No. 32361417, which I very much appreciate.
I'd like to order business cards and letterhead with Design No. 32361417 as soon as possible, but I want to make sure that you incorporate the correct design (attached). I'd also like to provide my information to add (address, phone number, email, fax) but I'm not quite sure how to do it.
