Flyer Design Brief

Hello,
I need a 8.5 x 11 size flyer design to highlight the differences between my Practice and other traditional primary care practices. The differences I'd like to highlight are explained in the attached file.

I'd like the designer to incorporate my logo (attached) and keep the aesthetics of the design similar to my logo colors; royal blue, gold and white.

I'd like an elegant design.

Please also include my name (Kamaljit Kaur, M.D.) and the contact info as shown in the rack card design (also attached.)
