This business card is to represent my medical practice, which is a Concierge Primary Care Practice, in South Florida (USA.)
I'd like the designer to leave a space for the Office Address & Phone Number as I have not yet chosen these items.
I'd like to have a QR code on the back of the card that links to www.TheLotusMD.com.
I'd like the theme of the card to match my Logo (which should appear on the front of the card in some way.)
Please include my name on the card "Kamaljit Kaur, M.D." with the Title of CEO.
The front of the card should only have the logo (Lotus MD) and be blue.

My target clientele is ages 25-64, upper-middle class/affluent population and both male & female.
