The description is in the uploaded file. Please let me know if you need any more information. I'd prefer it if someone could implement the design to a WordPress site but the best design will win, regardless.
I would like the forms to look nice as well
UPDATE:
I don't think we can use the images that I provided, I think they make the site look a bit 'cheap' and I really don't know how to replace them. One thought I have had is to make the "Simple, Unique, Effective" a second feature on the right side of the page with the search on the left
