Logo Design Brief

S.A.F.E.P.A.T.H. is looking for a logo that encompasses COMMUNITY - we have 5 or 6 community organizations coming together, in Monroe County specifically, to create a HEALTHY outcome for all our citizens! All generations, all races, all religions are included!

We will be working to make sure people can stay AHEAD of their HEALTH, and get the help they need for their current HEALTH - PHYSICAL AND MENTAL - needs.

Everyone should have access to transportation, housing and HEALTHY activities in the community.

We are also using ONLINE Health Risk Assessments to help people see where they can help improve their own health situations, and where they can get and ask for help


FROM THE GRANT:

1) to support recovery and resilience from both pandemic and longstanding trauma in the community for both individuals and families
2) to assess and encourage personal wellness and improved health status as a component of enhancing family health and integrations
3) to work with other community agencies to improve and increase wrap-around services, such as transportation, housing, and community involvement.
4) to increase overall county health awareness and improve both individual and group health status.
Specifically, the plan is to use several online health assessments to establish baseline health status for individuals. Each individual will receive counseling and awareness and develop his/her own health action plan. Chronic health concerns will be monitored closely – in the clinic, with home care, and in the community
