Logo Design Brief

We need a logo design for a specialized clinic / program we run for children with heart problems that we can help to manage and monitor from their homes. Also the HOME part is an acronym for:
High-risk
Outpatient
Management &
Education

This program is a collaborative between our company, Capital Pediatric Cardiology, and a company called Sutter Health.

Along with this description, I will attach the current Hearts at HOME logo (which is not good at all!), as well as our business Capital Pediatric Cardiology's logo and Sutter Health's logo. I think it would be best to have a Hearts at HOME logo that could stand on its own and not include the other two, but that in certain situations we could put the other two alongside as sponsors of this program.
