T-shirt Design Brief

I need a t shirt design with a cartoonish lizard with its tongue out wrapping around a piece of paper that is an application. The application would be called " Pit Lizard Application" The application needs to have an area for "Name" , "Hometrack" , and "Favorite Divison" The Favorite divison needs to be a check box style with serveral options, the options are "Sprint Car" "Non-Wing" "Midgets" "Late Models" "Big Blocks" "Other" At the bottom of the application the needs to be a spot that is for an Only Fans username with OF logo.
Colors are limited to green for the lizard, red for the tongue and application name font, black for the rest of the font, and blue for the OF font and white for the paper.
