CD Cover Design Brief

I want a CD cover for a single release that incorporates my photo. Simple classy and beautiful. I have attached my photo and a clip of the video. The artwork should include my name, Kenneth Little and the title of the song "Listen Everybody". I also would like to use this for other promos so in designing where you put the title of the song, it can be replaced with another title.
