Flyer Design Brief

Busy cardiology practice needs an order form for a heart scan test. Sample is attached. One side should have all information noted and the other side should have our physician names (Rakesh Shah, MD, B.T. Turakhia, MD, Snehal Patel, MD, Jinesh Shah, MD, and Francis Uricchio, MD. It should have the map that is uploaded and our demographic information (Bay Area Heart PLLC, 450 Blossom St. Ste. D, Webster, TX 77598 along with phone 832-905-5940 and fax 832-905-5941) as well as our web address (bayareaheart.com). Our primary colors are red, black and white. Needs to fit paper size 8 1/2 by 11.
