Is it will be Bi-Fold brochure with front and back cover page design and 2 inner pages which will have the content you provide in the MS Word file .

It should be an A4 paper evenly divided to 3 parts.
The logo has not been finalized yet, but likely will be the name of PhESi with blue color.
Other than that, you can have the freedom.

You ask for a tri-fold brochure but the text file you sent has lot of text and it will not cover in a tri-fold brochure you need a bi-fold brochure for that where we will provide you front cover design with logo and images related to services then we have 2 inner pages of content and then we can also use the back page as the content page.

Your text is not at all suitable for a A4 trifold brochure along with the diagrams you supplied. If you go for a A5 bi-folded to A4, that can give all of your text enough room with a considerable visibility. Please decide for which you will go asap.

Just a missed typo in my question above. It should be A3 to A4 instead of A5 to A4.

One more question, should I use the diagrams only given in the ppt or all the others you have attached in your word doc as well as the diagrams given in the ppt? However, I am making a clinical image collage for your front cover with my stocks.
