Business Card Design Brief

I ahve the logo in JPG format and now I am looking for a simple and elegant two-sided business card.

Back: APPOINTMENT should be centered on back of card. Underneath and flushed left should be Date:__________
Time: ____________
and then at the bottom of the back in slightly smaller font: Must cancel with at least 24 hours notice or the full-session fee will be charged.

Front of card should have logo displayed.

As you can see, I am missing the address and the domain for the email as I have got my office space or my website. I am just getting ready what I can at the moment so I would need to be able to add the address and domain at a later date.
