This rack card will go to attorneys, probation officers, and court affiliates to provide to clients with legal charges who need alcohol & drug assessments and/or treatment options.
Information needed on rack card:
- we provide alcohol & drug (A&D) assessments for adolescents and adults in a state licensed, private, modern setting located 6 minutes from the Clarksville Montgomery County courthouse and conveniently close to public transit
- low cost and one-time fee for an all-inclusive, comprehensive clinical A&D assessment which includes urinalysis drug screen, clinical recommendation summary, and fax delivery to 3rd parties (attorney, probation, DCS, etc...) per client request and with a signed release of information (HIPPA compliant).
- we follow American Society of Addiction Medicine (ASAM) criteria and National Institute of Drug Abuse (NIDA) guidelines to determine if recommendations for treatment, counseling, or education classes are indicated by the assessment.

- We also provide outpatient counseling and clinical support for clients struggling with substance abuse
- we offer referrals for detoxification, inpatient treatment, and other levels of care at no charge
- our staff includes licensed, masters level counselors that specialize in substance abuse as well as co-occurring mental health conditions such as PTSD, anxiety disorders, depression, ADD/ADHD, etc...

- we accept most major insurance policies and offer hardship rates with payment plans for our outpatient treatment options
- locally owned, operated, and managed by a Clarksville native
