Business Card Design Brief

I need a nice business card design that is double sided. This is for a dental surgery. I have already had a logo designed and just need the business card put together.

It needs to be in a format such that I can make new business cards if new dentists begin working for me. Hence I need to be able to change the text in order to have new names written on new cards.

The front will have the following text:

Dr Jalal Khan
B. Med. Sci. UNSW B.Dent. Syd. Uni

The logo will appear somewhere on the front. This is attached.

In addition will be the following writing that also needs to appear on the front of the card, perhaps at the bottom on the left side will be:

Shop 16, 105-153 Miller Street, North Sydney, NSW, 2060

and at the bottom on the right side will be:
(02) 9923 2478

On the back of the card will be a section for a map to be drawn (I have uploaded a picture) and also a section written as follows:

"Your next appointment is: (a line needs to be here to write date and time)"

Also I need two lines for two appointments to be written.
