Card Design Brief

Hi, I am a financial advisor and we do financial seminar for different groups of people such as students, professionals, Sr. Citizens etc. At the end of the seminar, I hand them an evaluation form to fill out. The goal of the evaluation form is to get their contact details, ask if they are interested in a complimentary consultation with me and my team, get feedback on the seminar etc.

We would like to create a nice card size evaluation form that we can hand out that is well designed, branded uniquely and leaves a good impression.

I am attaching an example of current form that we had created a long time back and hand drawn illustration of what we would like on the card form. The illustration is just to provide some direction on the content that we would like to see. Please feel free to do design without constraints.
