Fields: First Name*, Last Name*, Age, Zip Code, Email Address*, Daytime Phone, Evening Phone, Comments.
Then, there will be a "submit" button to proceed to next step of form. Just below the "submit" button, should be the following verbiage: "You will have the opportunity to upload photo(s) and provide other pertinent information in next step."
