Greeting Card Design Brief

I need a bookmark 2.5 x 8.5, full color, 2-sided, that lists a number of questions for married couples to ask themselves. The numbering is a little mixed up. Please make it look good.
My layout started with a different idea, but now would like it to be the shape of a larger bookmark.
