This is static html site. No server side language (php, jsp etc,) required. So form field does not have to be functional...

No, Just use the pdf and do whatever seems best. If I was a designer I would just use any free 1 pager template (just google cool one page html template), adjust the colors/layout/content a bit and create 2-3 little graphics in there :-) But that's my lazy approach
