Banner Ad Design Brief
This is NOT a banner design!!!! I just didn't know what else to put it under, and the area I need designed is normally where a banner would go. It is web design for the header area of my blog ONLY, so a fast, easy job. I am not a complete designer dummy, but this will take me too long to do on my own, as it's a outside my knowledge base.
I need a header like Christine Hassler has at the top of her blog at www.christinehassler.com, with both A PLACE TO PUT A WELCOME VIDEO, as well as A PLACE FOR AN EMAIL OPT-IN, and MY PHOTO PUT ON THE RIGHT LIKE HERS. Basically, I need it to be very similar to hers. I even have a photo of myself ready to go. I already am using a beautifully designed wordpress template by Elegant Themes, so I will just need design code etc. to go where the "AGGREGATE" logo and tagline is now at: http://corinstephenson.com. It will obviously have to be a good deal taller (height) then the space there now.

Here are the details:
1. I need my name in a neat font like hers (Corin Stephenson), something similar but unique would be great.
2. Instead of the tag line "The Complete Resource for 20-Somethings etc." I need it to say "BE the Change"
3. I need a place to put a youtube welcome video. You can use this video of me for now: http://www.youtube.com/watch?v=6Sw1ewnqPdY&feature=channel_video_title
4. Next to the video I need an opt-in form similar to hers. I already use Benchmark email marketing services (http://www.benchmarkemail.com/), so I likely need the wordpress plugin "Contact Form 7" to get the opt in to be there, but I do not know how to do this and connect it with my Benchmark account.
5. I want the email opt-in phrase "How to Say Goodbye to the 20 Something Crisis" to instead say "How to Green the World by Greening Yourself." Then it can say "Enter your name and email to receive your free green gift and begin transforming the world."
6. The portrait of me that I want used is at: http://corinstephenson.com/wp-admin/images/IMG_6570c.jpg
This can be cropped considerably to just above my head and below the bottom of my dress in the picture. Height should not be smaller than about 4.6 inches, so the height of the entire design can be about that. I have attached a screenshot image so you can get the idea of what I'm looking for.
7. Background behind my portrait will need to be matched to fit with the template background that is already at www.corinstephenson.com, but it should be a different color though to stand out, and hopefully shaded interestingly with different shades as hers is. (*Please also look at the theme preview at http://elegantthemes.com/preview/Aggregate/ to see the color and texture possibilities for the header you are designing so that it will match the template. There are several great color and texture overlay ideas/samples.)
8. I like the color of her video/opt-in area (an off-white/beige), but the background for the image should be a shade that matches both my photo and the background of the theme.
9. I do NOT want the "Life Coach, Author, Speaker" area, but I do want social bookmark images and they should go underneath my name and tagline as shown in the screenshot I have provided.
I can provide the chosen designer guest access to my wordpress site if needed. I can also provide the original template files from elegant themes through emailing a zip file I guess so you can show me what you can do. Just let me know what you need so I can see what you can do before I choose you!

Reason: I absolutely love the last 2 designs by both designers, and they just need the tiniest of changes for me to select one.
The photo is the one with my hair blowing and the sky in the background and I'm looking down. The dark shadow on my chest in the image needs to be retouched so it doesn't draw attention please. And the photo needs to be a vignette so some of the background shows, and it needs to be the placement that Kamiella has in her final submission (larger and inline with the bottom of the opt-in box).
Please just resubmit with these final requests and I will choose one of you.
