I work in Adobe InDesign and can make you a template for it. I could look into editable PDF, you'd probably need to have Acrobat Pro. And for really simple designs I suppose you could use Word too.

InDesign will give you the most bang for the buck. It can also do interactive PDFs if you want to get really sophisticated.
