So.. You want the same layout just to edit the images and text?

The layout of the PDF I provided was ok. The layout in the Word document looks a little amateur (because I did it), so you could use a similar layout as the PDF or you could do something different.

Designers: please do not use generic images eg of smiling doctors, I prefer something a little more artistic and subtle. I have added a powerpoint with some images - if you can use these images that would be good (ie if the quality is ok) - it will give you an idea of the types of images I am looking for.
