Loyalty Card design needed.

This is what CACTUS POINTS was looking for in their greeting card design

The loyalty card name is Cactus Points. We already have a logo and would need to have the Physical Card design. The card will be made of PVC plastic and not the kind where it is sandwiched between two transparent layers of plastic. The card will not have a magnetic strip. But will have a QR code at the back. Card number has 12 digits and is written as XXXX XXXX XXXX. 4x4x4 numbers with a space in between each 4 numbers. Please show an option of putting the cardholder's name along with the number.

At the back of the card will be the QR code, the website address, lets just say loyaltycard.com
