In house dental plan brochure

This is what a business in United States was looking for in their brochure design

I am wanting to market an in house dental plan where a patient can pay a yearly fee that will entitle them to all preventive care( cleanings, exams, fluoride,x rays) for free. After that any other work ( crowns, root canals, fillings, etc) will be discounted by 20%. I have a logo that I would like to incorporate and I will give you the numbers in the future for the brochure. The cost of the plan would be $200 yr for adults and $150 for kids 18 and younger.
